Your initial appointment will consist of a consultation explaining your diagnosis and treatment, as well as anesthetic options. Occasionally, surgery can be performed the same day as the consultation. However, a complex medical history or multi-staged treatment plan will require an evaluation and a second appointment to provide treatment on another day.
Please assist us by providing us with your dental and/or medical insurance information prior to your appointment. This will allow us time to determine benefit information in advance and to better assist you with processing any claims. Please provide the following information at the time of your consultation:
- Your surgical referral slip and any x-rays, if applicable
- A list of medications you are presently taking
- Photo identification and any dental and/or medical insurance cards for scanning into our system.
To expedite your treatment, you may have your dentist send us your referral and x-ray electronically.
IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian at the consultation visit.
Please alert the office if you have a medical condition that may be of concern prior to your consultation (e.g. history of chemotherapy or radiation, history of heart attack or stroke, artificial heart valves, joint replacements, pregnancy). Please also notify us if you are taking any blood thinners (e.g. Coumadin, Lovenox, Eliquis, Pradaxa, Xarelto) or if you have received any medicine for osteopenia or osteoporosis (e.g. Fosamax, Boniva, Actonel, Reclast, Prolia).
If your dentist or physician who referred you to us has taken x-rays, you may request that your doctor forward copies of them to our office. If 3D imaging or additional films are needed, they can be taken at our office.
